Job summary

Johnson Memorial Health Services is seeking a dedicated Certified Nursing Assistant (CNA) to provide direct resident care in our care center. The role focuses on daily living activities, dignity, and person-centered care in accordance with individualized plans.

You will work with licensed nurses, document observations, and participate in team care, safety protocols, and ongoing training. Prior LTC experience and Minnesota CNA certification are highly valued.

Qualifications

• Must have unrestricted Nursing Assistant Certification (CNA) active in Minnesota.
• High school graduate preferred but not required.
• Previous LTC experience with acceptable references preferred.

Responsibilities

• Assist residents with bathing, dressing, grooming, toileting, nutrition, mobility, and social engagement.
• Assist with meals and help residents eat; ambulate and position residents.
• Communicate with nursing staff; report changes in resident condition.
• Follow care plans and document ADL, mood, behaviors, nutrition.
• Attend monthly meetings and training; collaborate with team.
• Respond promptly to call lights and maintain safety.
