Responsibilities

• Provides personal hygiene care and documents observations for nurse in charge.
• Takes vital signs timely and reports abnormals.
• Assists with food and fluid intake and records intake & output.
• Assists with elimination and documents patient response.
• Maintains body alignment and repositions every 2 hours for comfort.
• Supports turning schedule to prevent pressure sores.
