Job summary

CHI St. Joseph’s Health of Park Rapids, part of CommonSpirit Health, is seeking a Clinic Licensed Practical Nurse (LPN) to support patient flow and care coordination in a busy clinic.

You will conduct proactive chart preparation, gather up-to-date information for patients seen in the facility, and collaborate with the clinical team to prioritize tasks and ensure smooth daily operations.

Qualifications

• Graduate of LPN education program
• LPN licensed in Minnesota (MN)
• Basic Life Support (BLS) certification from AHA

Responsibilities

• Conduct focused patient assessments and data collection
• Report changes to licensed healthcare providers as needed
• Participate in developing and modifying a care plan
• Implement delegated interventions within nursing plan of care
• Delegate tasks to other LPNs and UAPs as required
• Provide safe and effective nursing care delivery
