Job summary

McIntosh Trail Community Service Board seeks caregivers to assist individuals with mental illness, addiction, or developmental disabilities with daily living activities in home or daytime settings.

You will provide training in basic personal and social skills, coordinate with medical teams, and maintain client records. Responsibilities include transportation and adapting to changing client needs under supervision.

Qualifications

• High school diploma or GED and any combination of training and experience.

Responsibilities

• Arranges for needed medical care and provides needed services to assure that medical care is obtained.
• Cares for individuals and families during periods of incapacitation or convalescence, providing companionship, personal care, and help in adjusting to new lifestyles.
• Instructs clients/residents on daily living skills in one-on-one or group instruction and may advise on behavioral problems and daily decisions.
• Prepares and maintains records of client progress and services performed, reporting changes to the supervisor.
• Provides transportation for clients/residents to appointments and activities.
• Other duties as assigned by Supervisor.
