Job summary
Faith Home Healthcare LLC is seeking a compassionate Certified Home Health Aide (CHHA) to provide in-home care in Union County, NJ, in accordance with established care plans. The role focuses on daily living assistance, personal care, and documenting services for patients in their homes.
Responsibilities include health care services, medication reminders, mobility support, and companionship. English proficiency, a valid driver’s license, and willingness to travel are required; prior home health
Qualifications
- Proven working experience as home health aide.
- Ability to monitor vital signs and to collect specimens.
- Familiarity with basic nutrition and personal hygiene standards.
- Proficiency in English required.
- Nursing and health care administration skills.
- Valid driver's license.
- Must be able to travel to Union County.
Responsibilities
- Provide health care services in patients residences
- Perform domestic and household tasks
- Accompany patients to doctors office or to hospital
- Provide medication reminders.
- Assist with clients personal care activities
- Maintain patients care records and document provided services
- Assist patients with mobility and physical therapies/exercises
- Provide companionship and basic emotional or psychological support
