The client is currently at Turlock, CA Rehab and will need consistent non-medical support as he transitions back home.

Client Needs:

• Diabetic; unable to lift himself independently
• Recently had hernia surgery
• Ambulates with a walker
• Can toilet independently but needs assistance standing up
• Needs help with showers and restroom assistance
• Shower chair available

Schedule Options (Flexible):

• Choose one preferred set:
• Monday, Wednesday, Friday
• Tuesday, Thursday, Saturday
• Morning shift: 2–3 hours
• Evening shifts also acceptable (flexible based on caregiver availability)
