The role emphasizes building meaningful relationships, ongoing training, and collaboration with family members and the care team to ensure exceptional service and client well-being.

Qualifications

• Verification of certifications held (HHA/CNA), CPR Card, and CEU's.
• Experience as a caregiver in a home care setting or skilled facility.
• Must pass background checks (AHCA).
• Ability to communicate effectively with clients and their families.
• Compassionate and caring demeanor, with a strong commitment to providing exceptional care.

Responsibilities

• Provide quality in-home care to clients, including assistance with activities of daily living, medication management, and other personal care needs.
• Develop meaningful relationships with clients and their families, creating a sense of connection and community.
• Maintain accurate records of client care plans, ADLs, and outcomes using our Care+ smartphone app.
• Communicate with the office, family members, and other caregivers to ensure seamless care coordination.
• Participate in ongoing training and professional development to stay up-to-date on best practices and industry standards.
