Job summary

Chrysalis of Ogden, UT is seeking a dedicated caregiver to support at-risk adults and/or juveniles with developmental disabilities. This role requires the ability to work in safety-sensitive environments and follow pre-approved programs to make crucial care decisions.

Qualifications

• Must be at least 18 years old.
• Pass a criminal background check and LEIE per state.
• Pass required controlled substance tests.
• Ability to read, write and speak English.

Responsibilities

• Maintain punctual attendance and adhere to the work schedule.
• Provide supervision and support to individuals daily.
• Assist with medication administration as required.
• Plan activities to improve quality of life.
• Complete required documentation and reporting.
• Follow state policies and provider codes.
