Job summary

Adult & Child Health is seeking a Community Support Specialist (Housing Focus) in Indianapolis to help individuals find stability through housing and access to community resources. You will advocate, coordinate services, and support independent living in a hands-on, community-based role.

The ideal candidate brings a degree in social services or related field, or relevant experience, and can travel within the community while maintaining compassionate, client-centered care.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services or related field.
  • Experience with individuals with mental health or substance use challenges is a plus.
  • Valid driver’s license and reliable transportation; willingness to travel within the community.

Responsibilities

  • Help clients find, secure, and maintain housing.
  • Build and maintain relationships with landlords and housing partners.
  • Connect clients to healthcare, employment, education, and social resources.
  • Advocate for clients to ensure access to needed services.
  • Support development of independent living skills and coping strategies.
  • Coordinate services with treatment teams and community partners.
  • Document services timely and accurately; follow-up with clients and referral sources.
