Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a qualified district nurse to assess and plan patient care, deliver evidence-based interventions and monitor medications. You will work with patients, carers and the MDT to ensure safe, compassionate care in the community.

You will develop leadership skills, delegate to junior staff and build strong professional relationships, while benefiting from induction, supervision and flexible working options. A full UK driving licence is required.

Responsibilities

• Assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient, carers and other professionals.
• Deliver interventions that are evidence-based and monitor/ administer medications safely.
• Deleg ate and support junior staff, building leadership skills and clinical decision-making.
