Johns Hopkins Medicine in Laurel, MD seeks a Community Social Worker (CSW) to join the Community Care Team supporting residents with complex chronic illness. The CSW conducts psychosocial assessments, provides education, advocacy, counseling, and links clients to behavioral health services while maintaining a caseload and collaborating with CHN/CHW to coordinate care.
LSCW-C certification and CPR within 60 days are required; MSW degree and 2+ years in public/behavioral health are preferred.
The Community Social Worker (CSW) is a member of the Hospital's Community Care Team (CCT), a program that supports Howard County residents with complex chronic illness. The CCT helps clients connect with resources to better manage their conditions, improve health outcomes, and reduce preventable hospital visits.
The CSW completes psychosocial assessments, provides education, advocacy, counseling, and support, and helps clients connect to behavioral health services. This role maintains a caseload and works closely with Community Health Nurses (CHN) and Community Health Workers (CHW) to assess psychosocial needs and determine appropriate emergent behavioral health services. The CSW also supervises the CHW embedded in the Emergency Department.
Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
