Under general supervision of the Population Health RN Supervisor and the Clinical Director, works as a member of the Community Health Center primary care team, participates and coordinates whole health services to meet patient and organizational goals and outcomes. Serves as liaison in an integrated health model to coordinate medical/dental/community health services.

Experience:
Preference to candidate with at least two years’ experience in a medical clinic or healthcare setting.
Knowledge, Skills, And Abilities Required
Participates in the strategic goals for GVHC population health services that facilitates the patient-centred care model to achieve optimal health of our patient population.
Serves the GVHC patient population to coordinate resources for those identified as underserved or in need. Connects patients with appropriate sources and services.
Measures the interventions delivered in relation to the progress of the treatment and care planning for the patient.
Initiates and maintains contact with key community resource providers on a regular basis, to build positive relationships.
Updates GVHC leadership on community services available, improves access for referred patients and works together to best meet needs of shared clients.
Ability to apply the philosophy, mission and vision and policies and procedures of GVHC.
Demonstrates successful and professional techniques in interviewing, screening and assessing patients.
Ability to work under minimal administrative supervision while exercising a high degree of initiative, organize and manage tasks while maintaining flexibility.
Emulates successfully interpersonal communication as necessary to interact effectively and sensitively with outside agencies and specialists.
Establish and maintain effective working relationships in a culturally diverse environment, particularly when dealing with sensitive issues.
Work effectively with diverse populations and age groups.
