Job summary

Qualifications

• Bachelor’s degree from an accredited college or university or RN license.
• Must have reliable transportation with a valid driving record and ability to drive for work-related assignments.
• Experience with Setworks, Kronos, Therap and MS Office is preferred.

Responsibilities

• Manages a caseload of varying ages and disabilities to improve support quality and independence.
• Develops community relationships and links resources to match identified needs.
• Monitors and evaluates services with regular reviews and mandated reporting when needed.
• Completes documentation within three business days and maintains required billable hours.
• Keeps information up to date through training and knowledge enrichment.
