Summary: TheCommunity Resource Navigatorwill serve as a primary point of contact for healthcare patients, as well as the communities we serve, in an ambulatory care setting.They will support individuals and families entering the healthcare system andassistwithutilizationand retention of benefits and community resources. They will also focus on helpingeliminatebarriers to all JPCHC services.This position will float to clinics as needed.
