Job summary

Rocky Boy Health Center in Montana seeks a Public Health Nurse to deliver community-based nursing services on the Rocky Boy Reservation. The role includes health education, disease prevention, maternal and child health, chronic disease management, and home visits.

You will work with healthcare providers and leadership to address community health needs and improve outcomes. You will operate both independently and as part of a team, conducting home visits, developing care plans, and leading health

Qualifications

Current and unrestricted RN license in Montana.
ASN from an accredited program; BSN preferred.
Clinical nursing experience, preferably in community/public health.
Valid MT driver's license and ability to make home visits.

Responsibilities

Provide nursing care, health counseling, and education to individuals and families.
Perform independent home visits to assess health needs and risks.
Develop care plans and provide instruction on health practices.
Organize and implement health education classes for the community.
Participate in in-service education and quality initiatives.
Collaborate with supervisor on program planning and reporting.
Coordinate with physicians on orders and standing orders.
Identify health trends and implement community-level interventions.
Assist in grants administration and ensure compliance reporting.
Lead or contribute to QA projects and maintain records.
