Job description

Community Partnerships Coordinator (Mental Health)

Lorenz Clinic of Family Psychology | Southwest Twin Cities metro | Full time, in person

Lorenz Clinic is one of the largest private, clinician-owned mental health clinics in Minnesota. We treat the problems that hold mental health issues in place, not just the symptoms. We work with children, adolescents, adults, couples, and families across Carver, Scott, Dakota, and western Hennepin counties, from offices in Minnetonka, Victoria, Chaska, Prior Lake, Burnsville, Apple Valley, and Lilydale.

We are also a training institution. Practicum students, interns, and fellows train here under close supervision, which shapes how we work, including how we would bring you into this role.

Why this role exists

A referral is an act of trust. When a pediatrician, family physician, school counselor, hospital team, or therapist sends us a family, they are extending a relationship they have already built, and they deserve to know what happens next. This role exists to keep that trust, office by office, week after week.

This is a new role in a program we are building now. The way you do the work will shape it for whoever joins you later.

If you have done relationship stewardship, you already know this work

Maybe you have kept a stewardship calendar, managed a portfolio of donors or alumni, or kept volunteers coming back year after year. Maybe you have kept a clinic's referring offices close. Then you know the work: you reach out on a regular schedule, you remember what people told you last time, and you stay useful between asks. The difference here is the ask. It is never for money. It is for a referral, and we make it rarely and plainly.

A portfolio of your own

If you have been supporting gift officers, keeping the stewardship calendar for someone else's donors, this is the next step: a portfolio of relationships that is yours to hold. You will also arrange for our clinicians to come to the offices you serve: talks for their staff, evenings for their families, workshops for their therapists. You set them up; our clinicians teach.

What the work looks like

You will hold a portfolio of referring relationships in your area: clinics, practices, schools, and programs, each with a schedule of contact across the year. That means about 25 contacts a week, about half of them in person: calls in the morning, and visits to three or four offices most afternoons.

Most offices take months to send a first family. You will be measured on the work you do each week, not on referrals that haven't arrived yet. Your contacts are counted every week, and we review them together.

This is not a sales role. There is no quota and no commission. We do not pitch, and we do not oversell. We show up, describe plainly what happens when someone is sent to us, and then do what we said. The people who do this well keep a promise on a Tuesday when nobody is watching. They are comfortable walking into an office for the sixth time when nobody has called back. They take quiet satisfaction in a clean list and a finished follow-up.

What you bring to the offices you serve

We do not give gifts to referring offices. We bring things that help their patients and their staff:

  • This week's openings, including evening appointments.
  • A quick start for the families they send: we schedule on the first call, not from a callback queue.
  • The clinical summary, sent back with the family's permission.
  • A clinician who calls back when they have a question.
  • Talks, family evenings, and workshops by our clinicians, which you set up.
  • One-page handouts for the families they are worried about.
What you will do
  • Keep a portfolio of referring offices in your area on a contact schedule: calls most mornings, visits to three or four offices most afternoons.
  • Ask, at every visit, call, and voicemail, how we are doing and what we could do better. Most organizations never ask their referral partners either question. Record the answer the same day, bring the themes to a weekly review, and go back to tell the office what we did about it.
  • Keep each relationship's record current in our CRM: who places referrals, how they like to hear from us, what they asked for last time, and what we promised. No client information ever goes in it.
  • Stay useful between asks: bring each office something it can use, such as this week's openings, an answer to last time's question, or a resource for the families they are worried about.
  • Send the Monday availability bulletin to referring offices and care navigators.
  • Confirm each morning that every family referred to us the day before was scheduled on the first call, and raise it the same day if not.
  • Send and record the clinical summary our clinicians write for every referred client, with the family's permission: at intake, at any change in level of care, and at discharge.
  • Set up conversations between our clinicians and referring physicians, school staff, hospital teams, and therapists, and make sure they happen.
  • Bring the weekly count to a weekly review, and help produce the monthly report that ties new clients to the relationships that sent them.
Your first 90 days
  • First week: learn the referral list and how we keep it, make calls with the Operations Manager listening, visit offices together, and practice the two questions.
  • First month: about 25 contacts a week, about half in person, each recorded the same day.
  • By 60 days: every office in your area is on its contact schedule, and the themes of what offices tell you come to the weekly review.
  • By 90 days: you are drafting your area's section of the monthly report and setting up the first talks by our clinicians.
This role might not be a fit if
  • You want to sell, close deals, or earn commission.
  • You want to build campaigns, run social media, or plan events as your main work.
  • You are looking for a path into clinical work. This is not a clinical role.
  • You want to work remotely, or from a desk most of the day.
  • You need to see results quickly. This work pays off over months, not weeks.
Requirements

Required:

  • Two or more years of experience building and keeping professional relationships over time, and keeping records of them. Examples include development, stewardship, alumni or volunteer relations, community relations, provider relations, and referral or care coordination. Equivalent experience will be considered.
  • Daily use of a CRM, donor database, or similar contact-tracking system, and comfort with email and spreadsheets.
  • Clear, warm, professional communication in person, by phone, and in writing.
  • A record of follow-through: you keep a contact schedule without reminders, including when nothing comes back for weeks.
  • Ability to travel daily across your area, with visits to referring offices most afternoons.
  • A valid driver's license, a reliable vehicle, and proof of auto insurance.
  • Discretion with confidential information.
  • Successful completion of a background check.
  • Residence in or near your area of the southwest metro.

Preferred:

  • Experience holding a portfolio or a stewardship calendar in development, alumni relations, or volunteer coordination.
  • Experience in healthcare, schools, or social services, and familiarity with HIPAA.
  • Knowledge of the southwest metro's clinics, schools, and hospitals.
  • An interest in how families, schools, and medical offices work together, and in why that matters for a child's care.
  • A bachelor's degree, or equivalent experience.
You might be coming from
  • donor relations or stewardship
  • development, annual fund, or advancement
  • alumni relations
  • community relations or community partnerships
  • provider relations, physician liaison, or referral coordination
  • care coordination or patient navigation
  • school family liaison work
Pay
  • $55,000 to $62,000 a year, depending on experience
  • Mileage reimbursed at the IRS standard rate for travel between work locations
  • No quota, commission, or referral bonus. The work is judged on the relationships kept, not on a sales number.
  • Medical, dental, and vision insurance
  • Life insurance, and short- and long-term disability insurance
  • 401(k) with employer matching
  • Health Savings Account (HSA)
  • Paid time off and paid holidays
  • Employee assistance program
What it is like to work here

We are clinician-owned, and we are a training institution, so people here are used to being taught and to teaching. You will report to our Operations Manager, who will supervise you closely at first, on purpose, and then less as you take on the work, with a weekly one-to-one and a weekly review. The referral list, the materials, and the CRM will be in place when you start, so you will be running a program on your first day, not inventing one. You will see the numbers every week, and you will know what they mean.

Schedule and location

Full time, salaried, and in person; this role is not remote or hybrid. You will be based primarily at one of our southwest metro offices, with daily travel across the area. Start date in early November.
