Responsibilities

• Implement outreach activities aligned with hospital priorities and CHNA findings.
• Develop and maintain outreach calendar with events, programs, screenings, campaigns, and partnerships.
• Conduct outreach in community settings as a visible hospital representative.
• Build relationships with community organizations, schools, faith groups, businesses, and civic groups.
• Identify emerging community needs and barriers to care to improve health access.
• Plan and coordinate health fairs, screenings, workshops, and events.
• Coordinate event logistics: registration, venues, vendors, equipment, staffing, accessibility, promotion.
• Support health education initiatives to raise awareness of services and resources.
• Evaluate activities and recommend improvements from feedback.
• Assist community members in accessing healthcare services and resources.
• Support referral pathways between partners and hospital services.
• Collaborate with clinical, social services, marketing, and Foundation teams to support initiatives.
• Maintain regular communication with partners to ensure coordination.
• Collect and track documentation for outreach activities, benefits, attendance, referrals, partnerships, outcomes.
• Assist with data collection and evaluation for reports.
• Support grant-funded program implementation and reporting.
• Maintain databases and partnership records using approved systems.
• Create and adapt outreach materials and digital content.
• Coordinate with Marketing for culturally appropriate materials aligned with branding.
• Promote events through approved channels.
• Support Foundation activities including events, fundraising, donor engagement, and volunteers.
• Assist outreach efforts to strengthen awareness and support.
