Job summary

Sutherland Integrated Health and Social Care Team is seeking a compassionate Community Mental Health Support Worker to assist individuals with mental health needs in their homes and local communities across North Sutherland.

You will work closely with Community Mental Health Nurses and a multidisciplinary team to deliver person-centred, recovery-focused care, promoting independence and engagement with services.

Qualifications

• Experience supporting individuals with mental health difficulties.
• Ability to work in the community and in clients' homes.
• Strong communication and empathy, with ability to work in a multidisciplinary setting.
• Ability to work independently and as part of a team.

Responsibilities

• Support individuals with mental health needs in their homes and local communities.
• Collaborate with Community Mental Health Nurses and the multidisciplinary team.
• Assist with care plans, promote self-management and early intervention.
• Provide administrative support to the service as appropriate.
