Job summary

Signature Health Inc. seeks a Qualified Mental Health Specialist (QMHS) to work mainly in the community, developing and executing client treatment plans and providing transportation to access resources.

You will build relationships with clients, families, and the Signature Health team to ensure continuity of care. The role emphasizes collaboration with the treatment team, crisis intervention support, and documentation in client charts per Medicaid/ODMHAS/CARF standards.

Qualifications

• High School Diploma or equivalent required; Bachelor’s Degree in related social services preferred.
• Valid Ohio driver’s license and proof of insurance required at all times.
• AHA Basic Life Support (BLS) certification required.
• 1–3+ years of case management or relevant experience preferred.
• CDCA preferred.

Responsibilities

• Diligently develop, prepare and present treatment program material to clients.
• Coordinate transportation to access identified resources as part of treatment plans.
• Work primarily in community-based settings and client homes according to treatment plans.
• Maintain clear communication and professional boundaries with all clients.
• Serve as a subject matter expert and support in developing sober support networks.
• Collaborate with the client’s treatment team.
• Assist in educational, medical, housing, and daily living skills to help clients succeed in communities.
