Job summary

The Center for Human Services in Sedalia, MO is seeking a Licensed Practical Nurse (LPN) for a full-time position in an ISL setting to promote physical and mental health through nursing processes and compassionate care.

You will train direct support staff in nursing delegations, provide Level 1 medication administration training, document services, participate in on-call rotations, and support individuals after hospital or ER visits.

Qualifications

• Graduate of an accredited school of Nursing with valid LPN registration in Missouri or compact licensure state.
• CPR certification required.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide basic preventative, therapeutic and rehabilitative nursing care.
• Train direct support staff in nursing delegations.
• Provide Level 1 Medication Administration training to staff.
• Document services per Medicaid and DMH certification requirements.
• Assist with CIMOR, HRST and APTS referrals promptly.
• Attend ISL program meetings and participate in on-call rotations.
• Assess individuals within 7 days of discharge from hospital or ER.
• Develop plans to meet medical and health care needs of individuals.
