POSITION SUMMARY/PURPOSE

Under the supervision of the Manager of Community Services the Community Living Coordinators are primary implementers of our organization’s services, working on the front lines in our communities. As our professional representatives in the field, this role is critical to the overall success of the programs we provide. The Community Living Coordinator provides support through a person-centered approach to meet the individual’s medical needs while effectively working towards assessible resources, personal care and to maximize their quality of life. The comprehensive coordination can help empower individuals to be as independent and successful in their individual goals, community living, and life skills. The Community Living Coordinator provides hands‑on support in the instruction and development of community and daily living skills based upon the individual’s need. This includes but is not limited to personal care, domestic household services, medical oversight and coordination services, community activities, supervision within the home, state required reporting, completion of assessments, and other services based upon their individual needs. Services delivered include, but are not limited to in‑home supports, personal supports, respite supports, etc. based upon service contract and authorizations.

A minimum of 60% of weekly hours are in direct ratio provided in community.

ESSENTIAL FUNCTIONS

Health Care Coordination:

  • Works directly with Community Living Specialist (CLS) in ensuring accountability for the efficient implementation of individual specific programs, daily IPP goal implementation and tracking, and completion of medical appointments/medical needs.
  • Consults with the Easterseals nurse and behavioral consultant as required based upon the individuals plan and needs.
  • Perform other related duties as required.

Administrative:

  • Provides ongoing training to the CLS.
  • Monitors the development, implementation and documentation of individual plans (i.e. progress reviews, reports, summaries, etc.)
  • Completes all required documentation in Therap (an electronic documentation system) each shift and other methods of documentation as needed i.e. monthly mileage reports, incident reports, etc.
  • Establishes and implements effective program strategies to address all individual need areas.
  • Ensures that residential programs for individuals are developed, implemented and revised.
  • Audits and monitors quality and compliance requirements. i.e. QSR, CARF.
  • Provides on-site support/monitoring of staff and individuals as needed.
  • Assumes responsibility for implementation of individualized treatment plan goals, teaching strategies, and reporting assignments.
  • Manages medical appointment process and related tasks (i.e. schedule appointments, follow up, documentation, communication to stakeholders, etc.)
  • Provides transportation in personal vehicle (i.e. medical appointments with individuals as scheduled, community resources).
  • Communicates on a regular basis with the interdisciplinary team members and other stakeholders (doctors, pharmacy, guardian) and advocates as needed.
  • Assists with financial guidance and documentation (bills, budgeting, benefit application, etc.)
  • Attends and participates in meetings as directed by manager (i.e department, clinical support, etc.).
  • Perform other related duties as required.

Direct Support:

  • Provides face to face hands on supportive assistance enabling individuals to maintain living in their home and community including activities of daily living, personal care and other activities to promote healthy living in the individuals home and/or community.
  • Assists individuals with transportation needs i.e. accessing public or other forms of transportation, provides individual access to community resources through transportation in personal vehicle with destinations pre-approved and responsible management of the mileage budget as determined.
  • Assists/educates with personal care such as eating, toileting, bathing and dressing as prescribed, based on the needs and abilities of the individuals served, which can include hands on supports.
  • Assists/educates individual with household chores as needed.
  • Assists individuals in making personal choices and accessing community activities.
  • Implements behavior support plans, behavior guidelines, and appropriate documentation.
  • Implements medical/health protocols and required guidelines.
  • Teaches and enforces safety practices.
  • Facilitates virtual programming as needed.
  • Encourages the use of natural supports and fosters involvement in social and community activities while maintaining safety of the individuals supported.
  • Perform other related duties as required.

JOB KNOWLEDGE

  • This position may be authorized to have access to protected Health Information (PHI) to perform specific job duties.

SKILL REQUIREMENTS

  • Valid driver’s license, good driving record as deemed by the agency’s liability broker. Will be required to drive company van as needed to transport individuals served.
  • Must have a smart phone capable of completing electronic documentation shift notes in Therap, Outlook, ADP for time and attendance, EVV and other work-related applications as required (i.e. Skype, Facetime for communication with your manager).
  • Certification of CPR & First Aid, PMT, and all other required state trainings within required timeframe.
  • Must have personal vehicle to attend external meetings, site visits, etc.
  • Eligible to obtain DDS medication certification based upon program needs.
  • Strong organizational skills to manage priorities, meet deadlines and plan effectively.
  • Manage and maintain a delegated budget for service’s delivered (i.e. mileage, recreational budget).
  • Clear and effective communication skills.
  • Excellent problem-solving skills.
  • Self-directed with ability to prioritize workload.
  • Strong computer and tablet skills with knowledge of Microsoft Suite, i.e. Word, Excel, PowerPoint, Yammer, Email.

EDUCATION

  • Associates degree in human services or related field required, Bachelor’s degree in human services or related field preferred. CNA Certification preferred.

EXPERIENCE

  • At least one (1) of the three (3) years’ experience with Developmentally Disabled Adults required. CNA experience preferred. DDS (Connecticut Department of Developmental) residential and/or in‑home services experience preferred. Therap experience preferred.
