Job summary

Abode Care Partners in Denver, CO seeks a Community Liaison to promote hospice services to healthcare professionals, families, and community partners. You will be the face of our organization, growing awareness and referrals while representing compassionate end-of-life care.

You will build relationships with doctors and facilities, attend events, and manage outreach activities in our CRM. A healthcare marketing background and strong communication skills are essential to success.

Qualifications

• Bachelor's degree in a related field and healthcare sales or hospice outreach experience.
• Advanced degree or certification in healthcare marketing; CRM experience and referral networks preferred.
• Minimum of 2 years of healthcare marketing experience.
• Possess a current and valid state driver’s license; be able to provide proof of auto insurance.
• Demonstrate strong interpersonal and communication skills.
• Proficiency with healthcare and administrative software applications.
• Apply analytics and problem-solving capabilities in daily work.
• Manage time and tasks effectively with strong organizational skills.
• Work independently with minimal supervision and collaborate across teams.

• Attend community events to raise awareness and represent the brand.

• Work with the care team to ensure smooth patient referrals and quality care.
• Stay current on hospice care through training and self-learning.
• Complete reports and admin tasks on time; follow healthcare regulations.

Responsibilities

As a Community Liaison You will:

• Visit hospitals, clinics, and senior care facilities to promote our hospice services

• Build strong, ongoing relationships with doctors, nurses, and referral partners

• Attend community events to raise awareness and represent our brand

• Track and manage outreach activity in our system (CRM)

• Use market insights to plan and improve outreach efforts

• Educate families and professionals on hospice benefits and how we can help

• Work with our care team to ensure smooth patient referrals and quality care

• Stay current on hospice care through training and self-learning

• Complete reports, documentation, and other admin tasks on time

• Follow all healthcare rules and regulations

Qualifications

• Bachelor's degree in a related field and prior experience in healthcare sales or hospice outreach required

• Advanced degree or certification in healthcare marketing, and experience working with CRM systems and referral networks preferred

• Minimum of 2 years of healthcare marketing experience.

• Possess a current and valid state driver’s license

• Provide proof of auto insurance
