Job summary

Enhabit Home Health & Hospice is seeking a driven professional to serve as the relationship manager between the company and referral sources such as physicians, hospitals, and senior living communities. The role focuses on education, program requirements, and growth of referral networks to enhance patient outcomes and transitions.

The candidate should have a healthcare-related bachelor's degree or equivalent experience, plus strong communication and presentation skills.

Qualifications

  • Bachelor’s degree in healthcare administration, marketing, or related field or graduate of approved school of nursing, therapy or social work. Equivalent years of experience considered.
  • Highly preferred graduate of approved school of nursing, therapy or social work.
  • Two years of experience in healthcare business development, community relations, marketing, or sales.
  • Knowledge of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
  • Valid state driver license in current state of residence.
  • Automobile liability insurance as required by law.
  • Strong interpersonal, communication and presentation skills.
  • If Clinical: must have and maintain clinical license per state requirements.
  • Preferred experience in home health, hospice, post-acute care, or related healthcare services.
  • Preferred relationships with physicians, hospitals or senior living communities and other referrals sources.
  • Reliable transportation in good condition; ability to drive in all weather.
  • Fluency in English; strong written and oral communication; intermediate tech skills (MS Word/Excel).
  • In-office attendance is essential; remote work not allowed for designated full-time roles.

Responsibilities

  • Serves as the primary relationship manager between the company and referral sources (physicians, hospitals, health systems, senior living communities, and other healthcare organizations) to support referral growth.
  • Educates referral sources, patients, families, and community partners about services and program requirements to improve patient outcomes and satisfaction.
  • Navigate post-acute care transitions and promote quality care and patient/referral source satisfaction.

Responsibilities

Serves as the primary relationship manager between the company and referral sources including physicians, physician practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations or progressional. Supports referral growth by providing awareness and education of company services and program requirements to referral sources, patients, families, caregivers and community partners to help navigate post-acute care transitions with the goal of improving patient outcomes, supporting care transition, and promoting exceptional quality and patient / referral source satisfaction.

Qualifications
  • Required to have Bachelor's Degree in, healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work. Equivalent years of equivalent work experience is considered in lieu of degree requirements.
  • Highly preferred graduate of an approved school of nursing, therapy or social work.
  • Required minimum two years of experience in healthcare business development, community relations, marketing, or sales.
  • Required demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
  • Required to have and maintain a valid state driver license in the current state of residence.
  • Required to have and maintain automobile liability insurance, as required by law.
  • Required strong interpersonal, communication and presentation skills.
    • If Clinical:
    • Required to have and maintain clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
  • Highly preferred experience in home health, hospice, post-acute care, or related healthcare services.
  • Preferred previous experience using electronic referral systems and Homecare Homebase (HCHB).
  • Preferred established relationships with physicians, hospitals or senior living communities and other referral sources.
  • Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
  • Oral communication, written communication, fluency in English, active listening.
  • Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
  • Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
    • Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic
