Job summary

Roseman University of Health Sciences' GENESIS program seeks a Community Liaison to provide leadership, care coordination, and community engagement oversight within the Office of Community Health Innovation.

You will work with faculty, population health specialists, and community partners to mentor students, address social determinants of health, and improve access to services for underserved populations, including occasional Saturday visits.

Qualifications

  • MSW from an accredited institution required.
  • Licensed Social Worker (LMSW, LCSW, or equivalent) preferred.
  • Minimum of three (3) years of experience in social work, case management, care coordination, behavioral health, or related field.
  • Experience working with underserved populations preferred.

Responsibilities

  • Lead household engagement, care coordination and resource connection to improve outcomes.
  • Coordinate and monitor student household visits and experiential learning.
  • Develop and maintain partnerships with community organizations and stakeholders.
  • Evaluate programs and document outcomes for continuous improvement.

Job Summary

The Community Liaison provides operational leadership, care coordination, and community engagement oversight for the GENESIS program within the Office of Community Health Innovation (OCHI) at Roseman University College of Medicine. The role supports individuals and households experiencing social, economic, behavioral health, and healthcare access challenges through comprehensive assessment, resource navigation, advocacy, and coordination of community-based services.


Working collaboratively with faculty, Population Health Specialists, and community partners, the Community Liaison help mentor students in addressing social determinants of health, improve household stability, facilitate access to services, and support positive health and social outcomes for underserved populations. The position also supports student experiential learning through mentorship and participation in community-based care initiatives.


Essential Functions


  • Household Engagement & Care Coordination Provides technical assistance to faculty, students, and interdisciplinary teams in conducting household assessments, and developing support plans, and coordinate services that address social determinants of health, improve access to resources, and promote positive household outcomes.

  • Resource Navigation & Advocacy Facilitate connections to healthcare, behavioral health, social services, and community resources; advocate for individuals and families and support continuity of care across service systems.

  • Community Support & Crisis Response Provide guidance on population level intervention, and resource coordination for households experiencing social, behavioral, or environmental challenges, including crisis situations and barriers to care.

  • Student Engagement & Experiential Learning Coordinate and facilitate community-based experiential learning activities by scheduling and monitoring student household visits, guiding students in household assessment and culturally responsive engagement, and leading interprofessional clinical rounds that promote student growth and accountability, integration of community experiences, and professional development. In addition to weekdays, some household visits and outreach activities may be scheduled on a Saturday.

  • Community Partnerships & Outreach Develop and maintain collaborative relationships with community organizations, healthcare providers, government agencies, and other stakeholders to strengthen community engagement, expand resources, and address service gaps.

  • Program Evaluation, Documentation & Quality Improvement Liaison with program data scientists to maintain accurate documentation and program records, monitor household and program outcomes, support reporting and evaluation activities, and contribute to continuous improvement initiatives that advance program effectiveness and community impact.


Salary Range: $65,000-$71,000


Requirements


  • Master of Social Work (MSW) from an accredited institution required.

  • Licensed Social Worker (LMSW, LCSW, or equivalent) preferred

  • Minimum of three (3) years of experience in social work, case management, care coordination, behavioral health, community outreach, or related human services field.

  • Experience working with underserved, vulnerable, or high-risk populations.

  • Experience working with underserved populations preferred.

  • Experience working with Graduate students

  • Bilingual English/Spanish/Tagalog preferred.


Knowledge, Skills, And Abilities


  • Knowledge of social work principles, ethics, and professional standards.

  • Knowledge of trauma-informed care, crisis intervention, and behavioral health referral processes.

  • Knowledge of public assistance programs, housing resources, food security programs, and community-based support services.

  • Ability to conduct psychosocial assessments and develop individualized service plans.

  • Ability to engage effectively with vulnerable, high-risk, and culturally diverse populations.

  • Knowledge of family systems, community resources, and strengths-based intervention approaches.

  • Ability to manage complex cases while maintaining professional boundaries and ethical standards.

  • Knowledge of mandated reporting requirements and applicable laws governing vulnerable populations.

  • Proficiency with Microsoft Office Suite and ability to learn new systems and technology platforms.
