Job summary

NHSGGC in Glasgow is seeking a Band 6 Community Nurse in Learning Disabilities to work within a Multi-Disciplinary Team, managing a caseload and delivering expert assessment, diagnosis and treatment.

You will lead clinical governance, support staff, and collaborate with partners, families and local authorities to promote patient-centred care and health improvement for people with learning disabilities.

An exciting and challenging opportunity has arisen within Glasgow City HSCP Learning Disability Services (NW) for a Band 6 Community Nurse who is skilled and motivated to work within a MultiDisciplinary Team to improve the health of people with Learning Disabilities.

You will need to be clinically experienced but also have the knowledge and ability to manage clinical issues and staff governance on a daily basis. You must have the ability to support the operational demands of the service, monitor and respond to clinical governance issues and have excellent interpersonal skills to motivate and develop staff.

You should have a commitment to patient centred care and the support of people with learning disabilities. You will carry an appropriate caseload, providing expert assessment, diagnosis, evidence based treatments and management of this specific individual group and be available to give advice and offer expert opinion to parents, family members, carers, care providers, local authorities and other health care professionals.

You will be expected to support the Nurse Team Leader to deliver safe and effective care that enhances the patient experience and contributes to the delivery of Local and National Policies. You must have relevant post registration experience and professional registration.

Car Driver preferable given community position

All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern

• Location based in Glenkirk Centre
• Permanent Contract
• Full-time position (36 hours per week)
• Shift pattern for this post are 0900-1700 (30 mins meal break)

Key Responsibilities

• Have continuing overall responsibility for the management of own caseload.
• Be responsible for the specialist clinical nursing assessment, development, implementation, and evaluation of care programmes for individuals with complex physical and/or mental health needs within the service.
• To utilise specialist assessment tools and methods to facilitate evaluation of an individual’s status and plan appropriate specialist nursing service provision e.g. epilepsy care.
• Contribute positively towards a multi-agency/disciplinary approach to health care provision, maximising the quality of care provided and enhancing the quality of life experienced by the client group applying the principles of social inclusion.
• Develop and contribute to the health improvement of people with a learning disability, through health promotion activity and developing relationships with primary and secondary health partners.
• Managing the process of care coordination/case management including calling/chairing multi-disciplinary/multi-agency meetings and case conferences/reviews. This can include the responsibility for taking and distributing minutes to all relevant persons e.g. Health professionals/consultants, including non attendees e.g. GP’s, secondary care professionals.
• Demonstrate clinical leadership for junior nursing staff, acting as a resource for the nursing team within area of specialist expertise.
• Act as an expert nursing resource for clients, carers and other professional colleagues, advising on best practice and developing innovative practice.

Knowledge, training, qualifications and/or experience required to do the job

Essential

The post-holder will:

• Have 1st level registration RNLD.
• Have at least 2 years professional experience with evidence of proven leadership skills and competent technical, specialist and professional knowledge within the field of learning disabilities.
• Be able to display evidence of post registration educational development.
• Be flexible and adaptable in responding to service needs in a dynamic and rapidly changing environment.
• Display an ability to prioritise workloads and delegate appropriately.
• Be skilled in establishing and maintaining relationships with excellent interpersonal skills.
• Demonstrate analytical and judgement skills and competence in organisational and time management.
• Have motivational and team leading skills with the ability to advise, guide and negotiate.
• Have knowledge of current law and policy trends in learning disability and community care, including awareness of national and local policies, procedures and working practice.
• Possess excellent training and presentation skills.

DESIRABLE

The post holder should be:

• Computer literate.
• Educated to degree level.
• Competent in application of research based evidence in working practice.
• Be a car driver with a full GB licence
