The Arc of Prince George's County in Lanham or Largo is seeking a Community Integration Specialist to support individuals served in achieving greater independence and inclusion through person-centered planning, community activities, and daily living assistance.

The role involves transportation, documentation, staffing support, and collaboration with the Program Specialist, with hours 8:00 AM to 4:00 PM, Monday to Friday, at a rate of $19.17/hour (higher for bilingual Spanish speakers).

Position Summary:

This position reports directly to the Program Specialist. The Community Integration Specialist will provide the support necessary for the persons we serve to achieve their desired lifestyles and increase levels of independence, productivity, and social/physical inclusion into the community. This includes developing and assisting with the person-centered planning process, transporting persons supported to and from home and inclusive community activities, and supervising and implementing daily program activities and community experiences. Complete necessary daily documentation, as well as follow-up documentation.

Working Days & Hours:

• Scheduled Work Days: Monday -Friday
• Work Hours: 8:00 AM to 4:00 PM (40 hrs/wk)
• Location(s): Lanham or Largo
• Pay rate: $19.17 (Bilingual Spanish speakers receive a higher rate of $20.32)

What You'll Be Doing Day-to-Day:

• Preparation for Change : Involves planning for change, listening to individuals' interests, teaching essential community skills, providing support for success, and creating or reviewing Person-Centered Plans (PCPs) to ensure alignment with their preferences and goals.
• Community Integration and Participation: Involves helping individuals identify and plan community experiences based on their needs and interests, implementing Person-Centered Plans for inclusive activities and skill development, providing support for community engagement, teaching life skills, fostering positive work habits, and ensuring flexibility, including transportation, while promoting safety, dignity, and inclusion.
• Personal and Health care: Assisting individuals with daily living activities, providing constant supervision, ensuring personal and health care needs are met, maintaining a safe environment, responding to medical emergencies, administering medication accurately, adhering to safety protocols, and reporting incidents, while working in a respectful, professional manner and following all policies and procedures.
• Communication and Documentation:Completing accurate documentation, attending required meetings and trainings, supporting policies and the agency's mission, promoting teamwork and person-centered planning, actively participating in Person-Centered Plan development, managing challenging behaviors, and maintaining effective communication with team members, all while ensuring compliance with regulations and agency standards.
• Managerial Duties:Providing daily staffing support at various locations, instructing individuals in work-related skills, serving as a community liaison, ensuring professional and respectful communication, and safeguarding individuals' rights and dignity.
• General:Accurately documenting attendance, submitting behavioral data for review, completing regulatory reports on time, and maintaining proper documentation in the Electronic Health Record System for daily logs, PCPs, and Behavioral Management Plans.
