Imagine the Possibilities, Inc. is seeking a dedicated professional to support Day Habilitation (DAY HAB) and Supported Employment (SE) services in Jefferson with travel to Guthrie Center, IA.

This role focuses on community inclusion, skill-building, and employment opportunities for individuals served. You will collaborate with a multidisciplinary team to identify interests, develop schedules, and coach individuals toward independent task completion while following behavioral plans and ensuring

Qualifications

• Must be at least 18 years old and have a valid driver’s license.
• Reliable transportation and proof of automobile insurance.
• Education: high school diploma or equivalent.
• Strong communication abilities for teamwork.
• Willingness to learn and adapt in caregiving field.
• Ability to drive and transport for service provision if needed.

Responsibilities

• Collaborate with individuals to identify interests, goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and activities.
• Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction.
• Follow individualized behavioral care plans by applying approved strategies, encouraging participation, and supporting positive behavior.
• Maintain accurate, consistent, and timely documentation of individual progress and activities.
• Support individuals in job development and retention, including applications, interviews, coaching, and employer communication.
• Participate in cross-training, vocational program decisions, community integration activities, equipment safety, and ensure compliance with regulations.

This position will be primarily based out of Jefferson with travel required to Guthrie Center. It is a mixture of Day Habilitation Services and Supported Employment Services

This role operates in a Day Habilitation (DAY HAB) Setting, defined as services “that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment.” (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. It will also operate in a Supported Employment (SE) Setting, defined as services “that enables the member to obtain and maintain individual employment” (Iowa Health & Human Services). Rather than assuming individuals with development or intellectual disabilities are not qualified to work, Supported Employment services provide these individuals with the opportunity to obtain and maintain meaningful employment that supports their life goals.

• Person-Centered Community Engagement: Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
• Goal-Based Skill Development: Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction rather than doing tasks for them.

• Safety & Advocacy: Provide a safe, supportive environment while respecting individual choice and autonomy, advocating for independence, dignity, and person-directed decision-making in all aspects of service delivery.
• Vocational & Community Employment Support: Support individuals in job development and retention, including applications, interviews, coaching, employer communication, transportation to job sites, and collaboration with schools, IEP teams, and community partners.
• Program Participation & Professional Responsibilities: Participate in cross-training, vocational program decisions, community integration activities, equipment safety, medication administration (as applicable), compliance with all regulations (OSHA, DHS, CARF, HCBS), and maintain flexibility for scheduling, weekends, and additional duties.

Physical Demands:

• May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.

• May require prolonged standing for up to eight hours.
