Job summary

Community Options, Inc. in Manhattan, NY seeks a full-time Community Integration Counselor to support individuals in learning life skills and increasing independence within the community. The CIC will provide individualized services, assist in building community connections, and document progress toward integration goals.

Starting pay is $18.00 per hour. Applicants should have a high school diploma or GED (Bachelor's preferred) and a valid driver's license; background checks are required.

Qualifications

• High school diploma or GED is required; a Bachelor's degree is preferred.
• Valid driver’s license with a satisfactory driving record.
• Minimum of two years of experience supporting individuals with intellectual and developmental disabilities.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Employment requires successful background checks (criminal, central registry, child abuse, and drug testing).

Responsibilities

• Support individuals to increase independence.
• Meet waiver participants in residence, community, or office settings.
• Provide ongoing community integration support.
• Document progress toward community integration goals.
• Assist in building a support network between individuals and the community.
• Administer medications and ensure proper documentation.
• Coordinate leisure and volunteer opportunities.
• Monitor health and medical needs and report concerns.
• Transit individuals to medical appointments and activities.
• Ensure program documentation and billable records are accurate.
• Cooperate with license and department staff during inspections.

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.

We are now hiring a Full-Time Community Integration Counselor in Manhattan, NY. The Community Integration Counselor (CIC) is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.

Starting pay is $18.00/per hour

Responsibilities

• Support and encourage individuals to increase their ability to be as independent as possible
• Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
• Provide ongoing support for community integration efforts
• Document individual progress toward acquiring community integration
• Assist in building a support network between the individual and the community
• Administer medications and ensure proper completion of necessary documentation
• Coordinate all leisure and volunteer opportunities
• Monitor the health and medical needs of individuals and immediately report any concerns
• Drive individuals to medical appointments and activities in the community
• Ensure program documentation and billable records are completed accurately and timely
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

Minimum Requirements

• High school diploma or GED, Bachelor’s degree preferred
• Valid driver’s license with a satisfactory driving record
• Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities
• Ability to establish a comfortable and supportive relationship with individuals receiving supports
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
