Job summary

Community-Link- in Illinois seeks a dedicated professional to coordinate community living activities for adults with intellectual/developmental disabilities. You will plan weekly calendars, provide guidance on daily living, and help residents access community resources and supports.

The role requires a high school diploma or GED, a valid driver's license, and dependable transportation. You will work closely with program managers to ensure safety, documentation, and successful client outcomes.

Qualifications

• Must have High School Diploma or GED.
• Experience in residential settings is preferred.
• Must have a valid driver's license and auto insurance.

Responsibilities

• Coordinate planning and organizing community activities with Program Managers and Team Leaders.
• Develop weekly/monthly calendars of activities and communicate schedules.
• Guide individuals in daily living, personal care, and community integration tasks.
• Provide transportation for activities using agency vehicles.
• Ensure funds are available for staff and residents for activities.
• Follow safety procedures and complete required documentation.
