Job summary

ADAPT Community Network seeks a dedicated team member to provide ongoing training and personal assistance to Community Habilitation participants with disabilities, aiming to promote independence and community inclusion. Work with minimal supervision, delivering training, role modeling, structured routines and specialized support per each participant’s habilitation plan.

Activities are personalized to empower participants, with emphasis on autonomy, safety, and inclusion in community activities,

Qualifications

• Must be able to read, write and speak English.
• Proficient with Electronic Medical Record software.
• Able to write detailed session notes and follow prompts.
• Willing to travel to participant homes or community locations.

Responsibilities

• Adhere to agency mission and standards of conduct.
• Provide support in meaningful recreational, vocational, and daily living activities.
• Assess interests, abilities, and plan individualized habilitation.
• Follow habilitation plan dietary guidelines and safety procedures.
• Maintain confidentiality per HIPAA and privacy requirements.
• Collaborate with supervisors and circle of support for participants.
