The Community Health Worker (CHW) for the WIN Network primarily co-facilitates Group Prenatal Care (GPC) sessions, ensuring effective coordination, logistics, and participant engagement. CHWs provide targeted health education and coaching on prenatal care and healthy behaviors, conduct Social Determinants of Health (SDOH) screenings to identify and address barriers to care, and connect patients to community resources to support service utilization. Additionally, the CHW supports care coordination by serving as a liaison between patients and the clinical care team, promoting engagement and adherence to care plans.

Certification as a Community Health Worker or a willingness to become certified within six months of hire.
Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the Henry Ford Health Customer Service Policy
Must practice the customer skills as provided through on-going training and in-services.
Depending on the specific needs of assigned work, there may be variations of in person, hybrid or remote office-based work with frequent community outreach, including home visits and attendance at community events. Must have a valid State of Michigan driver’s license and automobile insurance.
