Job summary

Arab Community Center for Economic and Social Services in Sterling Heights, MI, is seeking Community Health Worker I & II to support wellness and social service referrals in local communities.

Under supervision, the role conducts SDOH screenings, health coaching, and health education, linking residents to Medicaid, housing, and social services while promoting preventative care and working with healthcare providers to improve outreach.

Qualifications

• High school diploma or GED required.
• Level I: 6 months to 2 years CHW experience.
• Level II: at least 2 years CHW experience.
• Valid driver’s license and reliable transportation.
• Current CHW certification preferred or willingness to complete.

Responsibilities

• Conduct SDOH screenings to identify needs.
• Refer clients to community resources and services.
• Engage with communities to promote health and access to care.
• Provide health education for chronic disease prevention.
• Document engagement and follow up on needs.
• Assist with navigating Medicaid, family planning, and medical homes.
