Job summary

ACCESS - Arab Community Center for Economic and Social Services is seeking a Community Health Worker I & II to conduct wellness consultations, including SDOH screenings, health coaching, and health education. Qualifications

  • High school diploma or GED is required.
  • Level I: 6 months–2 years CHW experience.
  • Level II: 2+ years CHW experience.
  • Current CHW certification preferred or willing to complete during onboarding.
  • Valid driver’s license and dependable transportation.

Responsibilities

  • Conduct social determinants of health (SDOH) screenings to identify needs.
  • Refer clients to community resources and other agencies.
  • Conduct outreach to promote health in the community.
  • Help clients navigate services and adopt healthy behaviors.
  • Provide health education for chronic disease prevention (BP, cholesterol, diabetes).
  • Provide health screenings (vital signs, HIV/test lead, POC).
  • Document engagement details with clients.
  • Follow up to determine if needs are met or further support is needed.
  • Assess eligibility for services (financial assistance, insurance).
  • Aid clients in obtaining healthcare services (Medicaid, care connections).
  • Serve as a liaison between communities and service providers.
  • Recruit new clients and expand referral partnerships.
  • Participate in continuing education and community events.

Job Title: Community Health Worker I & II

Job Status: Full-time

Job Summary: Under general supervision the Community Health Worker I & II will conduct wellness consultations that include Social Determinants of Health screenings, health coaching, health education, and referrals to social services available in local communities that provide valuable assistance to residents. Promotes preventative care and health screenings; works with health care providers to improve community outreach.

Essential Duties And Responsibilities
  • Conduct social determinants of health (SDOH) screenings to identify individual or family needs
  • Refer clients to community resources and other agencies to meet identified needs
  • Conduct outreach in the community that promote, maintain, and improve individual and community health
  • Help clients to navigate and access community services and adopt healthy behaviors
  • Provide health education for chronic disease prevention (i.e.., blood pressure, cholesterol, diabetes)
  • Provide health screenings such as vital sign intake, HIV testing, Lead testing, and Point of Care testing
  • Document details of engagement with clients
  • Conduct follow-up contacts with clients to determine if needs are met or additional support is needed
  • Assess client eligibility for services such as financial assistance, insurance, and other programs to assist the individual/family
  • Aid clients in obtaining healthcare services (Medicaid, family planning, connections to medical homes, etc.)
  • Serve as a liaison between communities, individuals, and service providers
  • Assist with recruiting new clients
  • Engage with community to identify referral partners and engage with existing referral agencies to strengthen partnerships
  • Participate in continuing education
  • Attend community meetings and health fairs to better understand community issues and provide information about agency offerings to community members
  • Operate standard office equipment and use required software applications
  • Perform other duties and responsibilities as assigned

Educational/Previous Experience Requirements
  • Minimum Degree Required:
  • High school diploma or GED equivalent
  • Level I – Entry level from 6 months to 2 years experience as a Community Health Worker
  • Level II - At least 2 years’ experience as a Community Health Worker
  • Candidate must feel comfortable interacting with active and recovering drug users (including individuals under the influence of substances at the time of interaction) recently incarcerated individuals, and unhoused individuals. The candidate will spend a significant portion of their time within treatment settings, including inpatient and detox facilities.
Licenses/Certifications
  • Licenses/Certifications Required at Date of Hire:
  • Current CHW Certification preferred or willing to complete during onboarding
  • Valid driver’s license and dependable transportation
  • Level I must complete 12-month apprenticeship program once CHWA certified
  • Level II MiCHWA training certificate
Working Conditions

Hours: Normal business hours, some additional hours may be required

Travel Required: Local, in-state, travel, up to 90%

Working Environment: Mobile unit and climate controlled office and/or remote work
