Job summary

Trinity Health Mid-Atlantic in Silver Spring, MD is seeking a part-time Community Health Worker to engage with local communities and connect individuals to essential health and social services. This role features a rotating shift (0.5 to 1.0 FTE) and a pay range of $22.83–$31.96 per hour, with compensation based on experience and education.

The CHW will educate patients, build relationships with faith groups and community organizations, and promote health literacy in one-on-one and group

Qualifications

  • Knowledge of the local community.
  • High school diploma or GED required.
  • Certified Community Health Worker preferred.
  • At least two years of community involvement.
  • Ability to communicate effectively in one-on-one and small groups.
  • Written and verbal communication skills; bilingual preferred.

Position Purpose

The Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has strong ties and a close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Responsibilities
  • Delivers services to vulnerable and high-risk individuals, particularly those hard-to-reach and/or with social needs, in home and community-based settings.
  • Build relationships within the community (i.e., faith organizations, community groups/centers, community leaders, Housing Opportunities Commission, community businesses, etc.)
  • Provides basic education regarding chronic health conditions and health promotion in a way that promotes understanding and self-management by the individual.
  • Reinforces health literacy and educational messages using appropriate tools and supplies within scope of practice.
Minimum Qualifications
  • Comprehensive knowledge of the local community.
  • High school graduate or GED required.
  • Certified Community Health Worker, preferred.
  • Minimum of two years' community involvement experience (i.e., community health worker, volunteer for community-based organization, outreach worker, community navigator, etc.)
  • Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization.
  • Ability to communicate verbally and in writing and to hear or communicate with target population, medical and social service professionals, colleagues, and peers.
  • Ability to inspire and motive others.
  • Motivational interviewing skills, preferred.
  • Proficient with Microsoft Office applications including Word, PowerPoint and Excel
  • Bi-lingual skills to work with patients in their native language are preferred.

Pay Range: $22.83 - $31.96 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

0.5 FTE (20 hours weekly) up to 1.0 FTE (40 hours weekly)
