Job summary

MedLink Health Center is seeking a Community Health Worker to coordinate care for Medicaid patients, helping remove barriers to equitable health outcomes. You will support enrollment, assist with re-enrollment, and connect patients to resources for insurance, food, housing, and transportation.

As a member of the Community Health Team, you will engage with patients, maintain confidential records under HIPAA, and participate in meetings.

Qualifications

  • Experience delivering patient-centered customer service in healthcare.

Responsibilities

  • Actively contact Medicaid patients to prevent un-enrollment and assist with re-enrollment.
  • Assist patients with financial barriers and align needs with available resources.
  • Follow up to ensure patients have active insurance, proper nourishment, housing, and transportation to appointments.
  • Answer calls promptly and provide information to ensure a positive impression of the organization.
  • Maintain confidential patient records in line with HIPAA guidelines.
  • Collaborate with Chronic Care Management staff to refer patients as needed.
  • Participate in team and staff meetings as required.
  • Other duties as assigned.

SUMMARY__________________________________________________________________________

The Community Health Worker promotes patient care for MedLink Health Center(s) by providing care coordination/case management and helping eliminate barriers to ensure patients will achieve a higher level of health equity. They will facilitate patients in overcoming social determinants of health barriers that patients face, such as transportation, food, housing, etc. As a recognized patient-centered medical home (PCMH), every employee is responsible for ensuring the successful implementation of quality, comprehensive patient-centered care as a member of the Community Health Team. By performing the job expectations described below you will be an active member of the care team, thereby patients are assured access to team-based, coordinated, comprehensive, quality care.

ESSENTIAL DUTIES AND RESPONSIBILITIES (include, but are not limited to, the following)
  • Actively contact and reach out to all Medicaid patients to help prevent Medicaid Patients from being un-enrolled and will help assist those with re-enrollment activities.
  • Assists patients with financial barriers and assists Medicaid recipients that may be facing re-enrollment challenges, as well as help align the patients' needs to other resources available.
  • Routinely follows-up with patients to confirm patient has active insurance, proper nourishment, housing, transportation to and from medical appointments and overall, is ensuring the patient is receiving proper care.
  • Answers all telephone calls promptly in a polite and professional manner.
  • Answers questions and offers other information, as requested, to provide patient centered service and a positive impression of our organization.
  • Maintains accurate and confidential patient records following HIPPA guidelines and regulations.
  • Works with Chronic Care Management staff to refer patients as needed.
  • Participate in team and staff meetings as required or requested.
  • Other duties as assigned.
QUALIFICATIONS______________________________________________

EDUCATION and/or EXPERIENCE__________________________________________________
  • High School diploma or GED AND a minimum of one year related secretarial experience in the health care field or high school diploma (or equivalency) with three years general secretarial experience.
  • Strong verbal and written communication skills
  • Strong computer skills with Microsoft Excel experience preferred.
  • Extremely dedicated in providing outstanding customer/patient service
  • Excellent organizational skills and attention to detail
