Job summary

Nascentia Health in New York State seeks a community health worker to engage with members, assess care plans, and coordinate support services. The role covers Oneonta, Utica, Madison, Eastern Onondaga, or Rochester regions, operating in a hybrid model with some travel.

You will collaborate with clinical and non-clinical teams, assist with Medicaid benefits and housing referrals, and help members navigate the health plan, ensuring timely communications and smooth care transitions.

Qualifications

• Associates degree in human services, LPN, or equivalent experience; high school diploma required.
• Strong computer literacy with MS Office and Internet.
• Valid NYS driver’s license with reliable transportation.

Responsibilities

• Travels to members’ homes to evaluate plan of care and needs every six months or as directed.
• Collaborates with clinical and non-clinical teams to update care plans.
• Orientates members to the health plan and reviews care plan including services and supports.
• Reports on members’ current level of care needs to clinical team.
• Refers questions to clinical and non-clinical team members.
• Conducts outreach to members missing monthly contacts.
• Assist with Medicaid benefits, spend-down, heating/food stamp, housing.
• Communicates with stakeholders in a timely and courteous manner.
• Participates in departmental meetings and trainings.
• Performs related duties as required.

Job description

Engages and provides peer support to group health members. Serves as a role model in maximizing wellness and independence. Collaborates with clinical and non-clinical team members to ensure a culture of support is reflected in members’ treatment.

This role supports Oneonta, Utica, Madison, Eastern Onondaga, or Rochester regions and their respective surrounding counties across New York State with a Hybrid work model. Applicant must live within the health plan service areas or surrounding counties and will support a hybrid work model with remote work and travel to plan member's residences.

Performance Responsibilities

1. Travels to members’ homes to evaluate their plan of care and ongoing needs for community health services and support at least every six months or as directed by Clinical Team.
2. Collaborates with clinical and non-clinical team members to ensure a culture of support in reviewing and updating the members’ care plans.
3. Provides support to health plan members, including orientation to the health plan, and the review of members’ care plan including services and supports
4. Reports on members’ current level of care needs in the community to clinical team members.
5. Refers appropriate questions and issues to clinical and non-clinical team members
6. Conducts outreach to members who are missing monthly contacts
7. Assist with entitlement initiatives such as Medicaid benefits, spend-down, heating and food stamp assistance, and housing
8. Communicate with internal and external stakeholders in a timely and courteous manner
9. Participate in departmental meetings and attend all scheduled staff meetings and staff training as applicable.
10. Perform related duties as required.

Requirements

1. Associates degree in human services, Licensed Practical Nurse, or equivalent of 1-3 years of relevant experience, with high school diploma or equivalency required.
2. Strong computer literacy and comfort, ability, and desire to advance technically and understand industry standards.
3. Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.
4. Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.
5. Valid NYS driving license required with own reliable transportation required.

Physical Requirements

• Ability to sit or stand at a desk 90% of the day
• Travel throughout NYS
• Visual/hearing ability sufficient to comprehend written or verbal communication
• Ability to express self verbally and in writing
• Ability to use telephone and communicate over the phone
• Frequently uses a laptop computer to enter orders and obtain product information
• Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids

[This position is an S01 exempt position with a min-max rate of: $62,353.20- $69,924.10/annually] with:
