Job summary

Waikiki Health in Honolulu is seeking a dedicated Community Health Worker to support high-need patients and coordinate care, education, and community connections to reduce ER visits and improve overall well‑being.

The role requires completion of a Medical Assistant/CHW program (or similar), at least three years of outpatient experience, a valid driver’s license, and CPR/BLS certification; benefits include health insurance, paid holidays, parking, retirement plan, and shirt.

Qualifications

  • Preferred Education: Completed Medical Assistant, Community Health Worker, or Health Educator program.
  • Preferred Experience: Three years of clinical work in an outpatient setting.
  • Mobility: Valid driver’s license and reliable personal transportation.
  • Interpersonal Skills: Able to communicate effectively with patients, staff, and community members.
  • Technical Skills: Proficient with Windows-based programs, typing, internet browsing, and email navigation.
  • BCLS: Valid Basic Cardiac Life Support certification, renewed biannually.
  • OSHA: Completed Occupational Safety and Health Administration safety training.

Responsibilities

  • Patient-Centered Care Coordination: Develop and implement tailored care plans and SMART goals for high-utilizer patient populations to reduce health disparities, improve outcomes, and decrease unnecessary ER/hospital visits.
  • SDOH Assessment & Resource Navigation: Administer the PRAPARE screening tool to identify SDOH needs, and actively connect patients to community resources, referrals, and program incentives.
  • Transitional Care Management: Ensure continuity of care by monitoring recently discharged patients and scheduling follow-up appointments within 7 days of ER/hospital release.
  • Cross-Functional Collaboration: Partner with healthcare providers, data analysts, insurers, and community agencies; manage cohort lists and present updates at IDT meetings.
  • Accurate EMR & Grant Documentation: Document encounters and vital stats in EMR within 24–72 hours and maintain SharePoint databases for grant reports.
  • Other duties as assigned.

Waikiki Health is seeking a full-time Community Health Worker.

Pay rate for this role is $22.00 per hour

Schedule: Monday – Friday, 40-hours work week

Summary of position: The Community Health Worker (CHW) position aims to improve outcomes for high-need, high-cost individuals by reducing avoidable emergency department reliance, supporting the quadruple aim of better care, lower costs, and enhanced experiences. Responsibilities include patient intakes, education, counseling, and population-based management within clinic protocols.

Qualifications and abilities

  • Technical Skills: Proficient with Windows-based programs, confident in typing, internet browsing, and email navigation, and demonstrated ability to master new software platforms rapidly.
  • BCLS: Valid Basic Cardiac Life Support certification, renewed biannually.
  • OSHA: Completed Occupational Safety and Health Administration safety training.

General Duties include but are not limited to

  • Patient-Centered Care Coordination: Develop and implement tailored care plans and SMART goals for high-utilizer patient populations to reduce health disparities, improve outcomes, and decrease unnecessary ER/hospital visits.
  • SDOH Assessment & Resource Navigation : Administer the PRAPARE screening tool to identify Social Determinants of Health (SDOH) needs, and actively connect patients to community resources, referrals, and program incentives (e.g., transit passes, hygiene kits, cell phones).
  • Transitional Care Management : Ensure continuity of care by monitoring recently discharged patients and proactively scheduling follow-up appointments within 7 days of their ER or hospital release.
  • Cross-Functional Collaboration: Partner closely with healthcare providers, data analysts, insurers, and community agencies; manage cohort lists and present updates regularly at interdisciplinary Team (IDT) meetings.
  • Accurate EMR & Grant Documentation: Document encounters and vital clinical statistics in the Electronic medical Record (EMR) within 24 hours to 72 hours and maintain SharePoint databases to generate required monthly and quarterly grant reports.
  • Other duties as assigned
