Job summary

Upper Great Lakes Family Health is seeking a Community Health Worker to serve as a liaison between patients and health and social services within the community. Responsibilities include identifying barriers to care, improving health literacy, and connecting patients to resources.

Qualifications include a high school diploma or equivalent, MiCHWA CHW Certification, and experience with an electronic health record system; may travel occasionally to other clinic locations.

Upper Great Lakes Family Health (UGLFH) is seeking a Community Health Worker to serve as a liaison between UGLFH patients and health & social services within the community.

Responsibilities include but are not limited to:

• Identifies and tracks patients with barriers to care i.e., transportation, financial, housing/shelter needs, etc.
• Identifies and tracks patients with health literacy issues and ensures the patient is provided with the appropriate resources.
• Provides patient education and connects patients to community resources.
• Partners with Care Managers to provide targeted interventions to avoid hospitalization and emergency department visits.

Job Qualifications:
