Job summary

Trinity Health in Grand Rapids, MI is seeking a Community Health Worker to assess and support clients across health and human services, helping them adopt healthier behaviors and self-sufficiency. You will coordinate care plans and provide client-centered services with professional integrity.

The role requires a high school diploma (Bachelor’s in human services preferred) and at least 1 year of related experience; bilingual Spanish speakers are highly valued for outreach and enrollment

  • 1 year of experience in health, education, or social services.

Responsibilities

  • Assess, plan, implement, monitor, and evaluate actions to meet clients' health and human services needs.
  • Coordinate and provide client support that is safe, timely, effective, efficient, equitable, and client centered.
  • Communicate program purposes and impact to clients.
  • Conduct intake interviews and enroll or refer individuals into community programs.
  • Assist health care providers in ensuring client understanding of care plans.
  • Develop and execute care plans; maintain electronic client files and reports; travel to client homes and community sites.

As a Community Health Worker, you will be assessing, planning, implementing, monitoring, and evaluating actions to meet with clients health and human services needs, so that they can adopt health behaviors leading to self-sufficiency. You will be coordinating and providing client support that is safe, timely, effective, efficient, equitable, and client centered.

Requirements for working as a Community Health Worker
  • High School Diploma or equivalent.
  • Bachelor’s degree in a human services field, preferred but not required.
  • Any combination of experience with health, education, and social services for a minimum of 1 year.
  • Basic understanding of health conditions, such as HIV/AIDS, Diabetes, COPD, Asthma, CHF, CVD
  • Bilingual - fluent Spanish speaking.
What Community Health Workers will do:
  • Communicating to clients the purposes of the program and showing them, the impact it may have on them.
  • Conducting intake interviews, enrolling and or referring individuals into community programs.
  • Assisting health care providers involved in the clients care, to ensure clients understanding of care plans.
  • Supporting the development, implantation, and execution of care plans.
  • Monthly reporting, maintaining electronic client files, including updated release of information, assessments, and other medical documents.
  • Travels extensively to client homes, community locations, agencies, and other outreach destinations.
