Job summary

Your Health is seeking a dedicated Community Health Worker (CHW) to service patients in the Athens, GA area, performing home and facility visits, delivering medications and DME, and coordinating care with providers. The role requires reliable transportation and a traditional M-F 8-hour shift.

The CHW will work with a physician group delivering primary care, specialty services, and pharmacy support across SC and GA, ensuring accessible, coordinated care and timely visits for patients in ALFs,

Qualifications

  • Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
  • Phlebotomy experience preferred.
  • Public health or home health experience preferred.
  • Experience with DME delivery or logistics is preferred.
  • Proficient with EMR and telehealth platforms.

Responsibilities

  • Work closely with care team to coordinate patient care.
  • Communicate effectively by phone, email, and in-person.
  • Coordinate visits with providers in homes and ALFs.
  • Deliver medications to patients' homes and reconcile instructions.
  • Coordinate telehealth visits between patients and providers.
  • Set up Remote Patient Monitoring (RPM) training and support.
  • Deliver DME to patients and train on proper use.
  • Perform routine venipuncture for testing and label specimens.
  • Support home-based care and diagnostics as needed.

We are looking for a dedicated candidate join our team as a Community Health Worker (CHW) to service our patients in the Athens, GA area.

The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services. Additionally, the CHW is responsible for delivering medications and durable medical equipment (DME) to ensure patients have timely and safe access to essential supplies.

Community Health workers perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working M-F 8-hour shifts (8:00AM-5:00PM)

What Are We Looking For?

A successful Community Health Worker will be able to perform these essential duties and responsibilities. Areas Of Responsibility
  • Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care plan.
  • Communicate effectively via phone, email, Microsoft Teams, Athena, in-person, etc.
  • Facilitate and coordinate visits with PT’s, OT’s and Providers in Assisted Living Facilities and patients' homes to enhance patient care and support.
  • Deliver medications to patients' homes, review instructions, and perform medication reconciliation, ensuring accuracy between current medication lists and what is found in the home.
  • Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers, ensuring patients receive timely care.
  • Set up training, instruct patients on the use of Remote Patient Monitoring (RPM) equipment, providing ongoing support, education and troubleshooting, when necessary.
  • Deliver durable medical equipment (DME) to patients in homes and ALFs, ensuring proper assembly and functionality. Provide training to patients and caregivers on the correct use of DME.
  • Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.
  • Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.
Qualifications
  • Certified Medical Assistant, Paramedic, or Licensed Nurse preferred. Certifications or licenses must be in good standing.
  • Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.
  • Previous experience in public health or home health preferred.
  • Experience with DME delivery or logistics operations preferred.
  • Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively. Strong written and verbal communication skills.
  • Basic computer knowledge.
  • Must be able to work independently with minimal supervision and direction.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.
  • Must have a clean driving record and experience operating vehicles in a professional capacity
