For nearly 100 years, the Visiting Nurse Association of Texas (VNA) has been a trusted pioneer in hospice care, helping our neighbors age with dignity and independence. Our mission-driven work spans 16 counties across North Texas—including Collin, Dallas, Denton, Kaufman, and Tarrant to rural communities in Fannin and Navarro-ensuring no one faces life's transitions alone. Job Summary

Working side-by-side with primary care teams, doctors, and social service agencies, you'll meet clients where they live and work - coaching them in managing chronic conditions, connecting them to vital resources, and empowering them to take charge of their well-being.

How You'll Make an Impact:
  • Guide & Coach Patients: Help clients understand their care plans, set personal health goals, and master self-care strategies for chronic condition management.
  • Break Down Barriers: Connect individuals to essential community resources, assist with program applications, schedule medical appointments, and resolve transportation or access challenges.
  • Build Trusting Relationships: Serve as a compassionate, culturally sensitive mentor to clients from diverse backgrounds, fostering confidence and self-advocacy.
  • Collaborate for Better Care: Partner directly with medical providers, nurse case managers, and local agencies to coordinate seamless, holistic support.
What We’re Looking For:
  • Education: Bachelor’s degree in Social Work, Human Services, or Psychology required.
  • Language Skills: Bilingual / Bicultural (Spanish) candidates are strongly encouraged to apply!
  • Community Focus: Passion for health equity, strong interpersonal communication, and a natural ability to build trust with diverse populations.
  • Requirements:
    • Verifiable good driving record, reliable personal transportation, and valid auto insurance for local community travel.
