Job summary

Neighborhood-Family-Practice is seeking a Community Health Worker to help patients navigate services and address social determinants of health. You will support care teams, conduct follow-ups, and assist with goal setting and referrals in the community.

You will document in the EMR within 2 business days of contact, maintain confidentiality, and collaborate with providers. Fluency in English and Spanish is strongly preferred, with Ohio CHW certification required.

Qualifications

• High School Diploma or equivalent required.
• Active CHW certification approved by the Ohio Board of Nursing.
• 1–2 years experience in community-based healthcare setting preferred.
• EMR experience preferred; EPIC EHR knowledge a plus.

Responsibilities

• Establish trusting relationships with patients and families and provide general support and encouragement.
• Provide ongoing follow-up, basic motivational interviewing.
• Follow up with patients via phone, home visits, and other settings where patients can be found.
• Help patients set personal health goals and attend appointments.
• Provide referrals for services to community agencies as appropriate.
• Help patients connect with transportation resources and provide appointment reminders.
• Record patient care management information in the EMR within 2 business days of contact.
• Manage assigned caseload of patients.

Position Overview: The Community Health Worker (CHW) is responsible for helping patients and their families navigate and access community services, resources, and adopt healthy behaviors. The CHW supports providers and the care team members through an integrated approach to care management and community outreach. Activities will promote, maintain, and improve the health of patients and their families by addressing social determinants of health (SDOH) that impact client health.
