Responsibilities

• Develop and coordinate community-based services for adults with serious mental illness.
• Assess mental health, co-occurring conditions, and functional needs.
• Utilize motivational interviewing in assessment and planning processes.
• Assist clients in obtaining and maintaining housing.
• Provide training related to personal, community living, and social skills.
• Support transition planning, discharge, and follow-up services.
