Job summary

Genesee County in New York seeks a professional nurse to provide nursing services countywide under supervision. The role includes developing and evaluating care plans, and coordinating with physicians to meet medical and psychosocial needs.

The position involves performing 18–20 visits weekly, acting as Case Manager for a client caseload, maintaining records, and participating in quality assurance and ongoing education while promoting high nursing standards.

Qualifications

• Valid NY RN license required.
• Valid NYS driver's license required.

Responsibilities

• Assess nursing care needs and implement care plans.
• Performs 18–20 client visits per week.
• Serve as Case Manager for a caseload as prescribed by Public Health Nurse.
• Collaborate with physicians to formulate a plan of care addressing medical, psychosocial and safety needs.
• Make referrals to other disciplines and county entities to maximize client health/independence.
• Carry out therapeutic treatments as ordered by the physician.
• Instruct patients and families on care at home and rehabilitative measures.
• Maintain client records and documentation for Medicare/Medicaid/Third party payers.
• Participate in quality assurance, continuing education and other requests.
