Qualifications

• Bachelor's degree in Nutrition, Dietetics or related field required on hire.
• Master's degree in Nutrition, Dietetics or related field preferred.
• Completion of ACEND-accredited internship required on hire.
• RD credential within 6 months of hire.

Responsibilities

• Develop and implement nutrition education and programming for individuals and groups.
• Provide nutrition counseling on chronic disease prevention, healthy eating, maternal and child health.
• Lead or support community programs like diabetes prevention, weight management, and school wellness.
• Partner with local agencies to expand access to nutrition services.
• Create culturally appropriate education materials and outreach events.
• Address social determinants of health impacting nutrition (food insecurity, housing instability).

Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

• Develop and implement nutrition education and programming, including individual and group assessments, and cooking demonstrations, and other services tailored to community needs.
• Provide nutrition counseling and education on topics such as chronic disease prevention, healthy eating, maternal and child health, and food access
• Lead or support community-based programs such as diabetes prevention, weight management, senior nutrition, and school wellness initiatives
• Partner with local agencies, clinics, schools, food banks, public health, and other departments to expand access to nutrition services
• Develop culturally appropriate education materials, presentations, and outreach events with a focus on improving health literacy
• Identify and address social determinants of health that impact nutritional status (e.g., food insecurity, housing instability)

Job Requirements

Required

• Bachelors Of Science in Nutrition, Dietetics, or related field, upon hire or
• Masters Of Science in Nutrition, Dietetics, or related field, upon hire
• Completion of ACEND-accredited dietetic internship, upon hire and
• Registered Dietician, within 6 - months

Preferred

• 1-3 years 1-3 years of experience in community or public health nutrition, upon hire
• Experience with Cerner or Epic, upon hire
