VNS Health is seeking a Community Health Nurse to provide comprehensive nursing care in a community setting in New York. You will perform health assessments, administer medications, implement physicians' orders, and coordinate care with onsite and community providers.

• Current RN license to practice in New York State (NYSRN) and CPR/BLS certification are required.
• Associate's Degree in Nursing is required; BSN preferred.
• Minimum two years of RN experience in LHCSA or CHHA is required; behavioral health experience preferred.

Responsabilidades

• Collaborates with interdisciplinary teams to coordinate health and wellness activities.
• Conducts client assessments to determine health needs, including vital signs as needed.
• Accurately documents assessments, interventions, and outcomes in clients' medical records.
• Provides emergency care until EMS arrives and liaises with EMS personnel during medical emergencies.
• Develops and implements individualized nursing care plans.
• Health Promotion: Educates clients on disease prevention and health maintenance.
• Assists in preparing statistical reports for funding sources (e.g., numbers of health assessments and referrals).
• Care Coordination and Advocacy: Facilitates referrals and linkages to appropriate healthcare services.
