Valley Health System in Blacksburg, VA is seeking a Community Health Nurse to coordinate and deliver personalized skilled nursing care at patients' homes. You will assess health needs, develop care plans, provide wound care and catheter changes, and manage medications while coordinating with a multidisciplinary team.

You will monitor progress, educate patients and families, and connect individuals with community resources. A valid nursing license and CPR are required; BSN preferred.

Qualificações

• Graduate from an accredited School of Nursing. BSN preferred.
• Computer and keyboard skills. Medical terminology required.
• Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing).
• Current and valid CPR certification.
• Demonstrates effective interaction and communication (oral, writing, presenting) skills.
• Ability to work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure.
• Ability to function competently in stressful situations and a changing work environment related to changing customer needs.
• Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, etc.).
• Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations.
• Ability to use critical thinking and clinical reasoning skills to effectively problem-solve and deliver care.

Position Summary

A Community Health Nurse is responsible for coordinating and providing personalized skilled nursing care to patients in their homes. This role involves assessing patients’ health needs, developing and updating individualized care plans, providing skilled clinical services such as wound care, drain management, catheter changes, medication management, and other nursing treatments. In addition, conducting case conferences with a multidisciplinary team that may include therapists, home health aides, dietitians, social workers, nurses, physicians, and other healthcare professionals. The Community Health Nurse monitors patient progress, coordinates services, educates patients and families about medications, treatments, and home safety, connects individuals with community resources, maintains accurate documentation, and ensures that care is delivered safely and effectively. Strong clinical knowledge, communication, organization, and care-coordination skills are essential for promoting patient wellness, independence, and quality of life.

Education

Graduate from an accredited School of Nursing. BSN preferred.

* Driver s License - current and valid driver s license, registration, inspection sticker and appropriate level of insurance coverage required, to include collision. Pay Range: $99,424.00 - $124,280.00

The posted pay range reflects the compensation for a full-time equivalent (1.0 FTE)
