Job summary

The Ponca Tribe of Nebraska invites applications for a Community Health Worker - RHTP position in Omaha, NE. This full-time role focuses on outreach health care services and health promotion to Native Americans within the Ponca service areas.

You will conduct home visits, coordinate care, facilitate referrals, and provide health education while building trust with community members. A strong background in public health and ability to travel are preferred.

Qualifications

• Associate’s degree in health or related field.
• One year of health-related setting experience.
• Primary Care – Pediatrics, Adult and/or Geriatrics
• Dental services
• Behavioral Health services
• Pharmacy
• Electronic Health Record (EHR - APRIMA)
• Population/Public Health
• Computer proficiency with Microsoft Word, Excel, PowerPoint and Access programs.
• Excellent written and oral communication skills.
• Valid driver’s license and be insurable.
• Able and willing to travel.

Responsibilities

• Assist the Population/Public Health Department in developing, implement, and expand community based healthcare services.
• Provide extensive outreach to Native Americans located in the Ponca service areas, educating members.
• Conduct Native American public gatherings to train in safety, nutrition/dietetics, environmental health, and other health concerns.
• Provide in-home visits to monitor healthcare services for patients with chronic conditions.
• Facilitate communications between community members and health care providers to improve access to care.
• Develop a referral network to meet health care needs and identify gaps in care.
• Design and implement health education programs providing primary to tertiary care.
• Serve as patient advocate and care coordinator for appointments and transportation.
• Maintain confidentiality and adhere to program standards.
• Network with service providers and partners across levels of government.
