Job summary

Avera Health in Sioux Falls seeks a Community Health Worker to connect patients with health and social services and to improve cultural competence in care delivery. You will support navigation, education, and coordination to enhance access and outcomes.

Responsibilities include health promotion, education, language assistance, and addressing social determinants of health for diverse communities served.

Qualifications

• High School or GED required.
• CHW Certification completed within state guidelines.
• Driver's License required.

Responsibilities

• Health system navigation and resource coordination for patients (appointments, transportation, access to services).
• Health promotion and coaching to improve health status (nutrition, tobacco, substance use, fitness).
• Health education to teach proven methods for avoiding illness and reducing impact.
• Provide culturally appropriate health information and translation assistance when needed.
• Educate on social determinants of health and ensure family members understand health information.
