Placements24 is seeking a Community Health Navigator to support residents in East London within a hybrid role. You will guide individuals to access healthcare services, educate on health resources, and help overcome barriers to care.

The role requires strong communication, compassion, and the ability to work both remotely and in-person outreach within the Eastern Cape. You will collaborate with local providers and community partners to promote wellness.

About the Role

This hybrid role involves assisting individuals and families in accessing healthcare services and promoting health and wellness within the community. You will play a key role in educating residents about available health resources, facilitating appointments, and providing support to overcome barriers to care.
